At Rooted Health and Wellness, our goal is to restore health in whatever way that means for the patient. Each patient is treated as an individual with a customized plan. For metabolic health, this may look like just a medication for one person or multiple labs with medications, diet, and exercise plans for another. Our goal is to meet the patient where they are at in their journey of improving their health and longevity with sustainable changes.

Flexibility
If you don't want the deep dig and labs and are only interested in medications, for example, we are flexible. If you want to use your insurance for some medications, we can change pricing to reflect that. Please contact us and we can create a specific plan/payment option for you.

Individualized Care
Every patient has different needs and goals. Every plan is individually tailored to meet this. We do not follow algorithms for care for any issue. For example, we don't automatically increase doses of medications just because we can. We make decisions based on patient tolerance and response. 

Relational
Forget the 5 minute telehealth video visits with large scale companies or the 5 minute office visit to address a chronic symptom with your primary care. We are all about building the relationship between you and the provider.

Our History

Founded in late 2023, Rooted Health and Wellness was dreamed about then brought to fruition after struggling to break through barriers of conventional healthcare, including insurance and inadequate time, to improve patient care. It was also started due to wanting to help people feel better and helping them dig into the cause behind why they aren't.

Carlie Johnson is a local nurse practitioner. She graduated with her nursing degree from University of Southern Indiana in 2012 and moved to Columbus after graduation where she mainly worked in the ICU as an RN until she graduated with her FNP from Frontier Nursing University in 2016.

Through her career, she has found significant issues that unfortunately can't always be addressed through conventional healthcare, notably working to get to the root cause of health conditions of her patients, often due to time and insurance constraints. She is excited to start help optimize the health of the community in another way.

Carlie Johnson, FNP-C

Meet Katie - an ACE Certified Personal Trainer who’s passionate about helping you build a positive and approachable relationship with movement. Born and raised in Columbus, IN, Katie brings real-life relatability and energy to her coaching. When she's not in the gym, you'll likely find her spending time with her fiancé and their three cats or coaching middle school girls' basketball.

Katie transitioned into the fitness world in 2021 after a career in higher education, and she’s found her true passion in supporting those who are brand new to the gym or unsure where to begin. Her goal is simple: to help you feel confident, capable, and comfortable moving your body—wherever you’re starting from. Katie creates personalized fitness plans that fit your life, meet you exactly where you are, and support your goals without pressure or overwhelm.

She’s proud to be part of the Rooted Health & Wellness team, working alongside Ada and Carlie to provide well-rounded support for your health and wellness journey.

Katie Barker

Meet Ada - a certified nutrition coach with a passion for holistic health, food freedom, and body image. Originally from Poland and now a mom to two dogs, two cats, three chickens, and one very energetic baby, Ada brings both life experience and professional insight to the table. She loves exploring the outdoors, diving into a good book, and discovering the best local food and coffee spots.

At Rooted, Ada is here to help you implement personalized nutrition protocols that align with your lifestyle and goals. You'll get the accountability and support you need to make lasting changes, not just in what you eat, but in how you think and feel about food and your body. Her approach goes far beyond the plate, integrating confidence coaching and subconscious reprogramming to help you overcome challenges like food obsession, emotional eating, and restrictive patterns.

Meet Nicole - Our clinic coordinator.
Hi, I'm Nicole. I’m originally from Colorado and a proud pet parent to two fun-loving fur babies. I met my husband in Idaho where we both attended school. After graduating, we moved to Indiana and have been here ever since.

I grew up playing sports and later coached swimming after college - I loved being able to support, teach, and work with kids. After moving to Indiana, I was fortunate to be able to follow my passion for creativity and connection by attending cosmetology school. While my career path has taken a few turns, one thing has stayed the same: my passion for helping people.

Now, as a Clinic Coordinator, I’m grateful to be in a role that allows me to support patients and providers behind the scenes. I’m always on the go, so while you may not always see me in the office, I’m still here to help and make sure everything runs smoothly. In my free time, I love traveling with my husband and our dogs—exploring new places together is one of my favorite ways to recharge and create lasting memories.

Nicole 

Brittany is a local family nurse practitioner who graduated with her nursing degree from Indiana University school of nursing in Columbus in 2015. She has lived in Columbus her whole life. After graduation she worked in the ICU as an RN at Riley Hospital followed by outpatient triage nurse at a family practice clinic in Carmel, IN while she pursued her Master's. She graduated with her FNP from Chamberlin College of Nursing in 2018. She has worked at the local gastroenterology office and continues to do so, but has found throughout her career, significant issues with only traditional medicine approach to problems. She is excited to offer a more comprehensive approach to patients in the community.
